Welcome to Chicago Cosmetic Surgery & Dermatology. We’re excited to welcome you to your appointment. Below you’ll find all the essentials to help you prepare, navigate, and feel confident about your care — from registration and insurance to parking and policies.

Registration & Paperwork
- Use the Patient Portal to complete all required forms remotely before your visit.
- If you can’t access the portal, you can view & download the forms below to complete and bring to your appointment.
- If the patient is under 18 and attending alone or with someone other than a parent or guardian, a Treatment of Minor Consent Form must be completed in advance.
- A Medical Records Request Form is available if you need to transfer or release records.

Getting Ready for Your Appointment
- Scheduling: Call us at (312) 245-9965 (option 2), schedule online, or request an appointment via the Patient Portal.
- Arrive Early: For first-time visits, arrive 15 minutes early (30 minutes if completing forms on-site).
- What to Bring: Valid photo ID, insurance card, referral (if required), and completed forms (if not submitted in advance).
- Payment: Insurance co-pays or patient responsibility amounts are collected at the time of service. Cosmetic and self-pay fees must be settled before treatment.
- Cancellation: We understand that things happen that may interfere with your appointment. If and when that happens, please contact us as far ahead of time as possible (at least 24 hours or more). For medical dermatology appointments cancelled within 24 hours of appointment, a $25 fee is assessed. For cosmetic dermatology, aesthetic services, or plastic surgery appointments cancelled within 24 hours of appointment, a $50 fee is assessed

Insurance & Financing
- You are responsible for providing your current insurance information upon scheduling. Insurance will typically apply to medical dermatology, but does not typically apply to cosmetic dermatology, esthetic services, or plastic surgery.
- Verify that your provider is in-network to avoid unexpected costs.
- For cosmetic treatments, we have Financing Options available.

- During your consultation, we take your medical history, evaluate your skin, and discuss goals and options.
- Pre- and post-treatment guidance is provided to support your safety and results.
- Treatment costs vary depending on the procedure, area treated, and your goals.

Parking & Location Information
- Parking Garage: Use the North Bridge Garage at 10 E Grand. Bring your ticket—our team will validate it, and you pay $10 for up to 4 hours. Note: We no longer validate for the Grand Plaza Apartments lot.
- Rideshare Tip: If using Uber or Lyft, set your destination to 515 N State St. to reach our entrance easily.
- We are located on the 9th Floor of the FiveOneFive North State building. There are two sets of elevators—you will take the set labeled 2-17.

Traveling From Outside Chicagoland
Many of our patients travel from across the country to see our expert providers. Our centrally located downtown Chicago practice is easily accessible by car, train, and air—with both O’Hare International and Midway airports just a short drive or ride away. From the CTA train lines to Uber and Lyft rideshare options, it's easy to travel to and from our clinic and surgical center. Whether you’re visiting for a single treatment or an extended plan of care, our team is happy to provide guidance on nearby hotels, dining, and transportation to make your visit as smooth and comfortable as possible.

You can complete our New Patient Registration Form online before your visit to save time at check-in. If you prefer, you may also fill it out in-office on the day of your appointment.
